Seeking: Public representatives for transformation care workstreams

A partnership of health and social care organisations in Hackney and City is seeking public representatives for four groups (called care workstreams) that will join-up health and social care services and improve health and wellbeing outcomes in the following areas:

  • Unplanned care
  • Planned care
  • Prevention
  • Early years and children 

The public representatives will play an important role ensuring the work carried out in each care workstream includes service user perspectives. These roles will contribute to ensuring residents are at the centre of the local health and care system.

About the partnership

Hackney & City: Health and Social Care Transformation is a partnership to improve health and wellbeing outcomes for Hackney and City of London residents through closer working and integrating services between local health and care organisations. The partners include City and Hackney Clinical Commissioning Group (CCG), Hackney Council, Homerton Hospital and Healthwatch Hackney.

We are aiming to have two public representatives on each care workstream. Interested candidates should submit a completed application form, personal statement, CV (no more than 2 sides A4) and equality form to info@healthwatchhackney.co.uk for the attention of Jon Williams. The closing date is 5pm Friday 5 May. Appointments will be made following a short panel interview against the person specification in the week 8-12 May.
